What can you think of as embarrassing/awkward/bad mix ups. I can think of a couple off the top of my head.

When I was younger (say 9) I asked my maybe 3 year old cousin if he wanted to watch Noddy and put the tape on for him then left the room (being too old for it). About 15 minutes later my aunt came in very annoyed that I was, of all things, letting him watch Chubby Brown. Turns out someone had put it back in the wrong box! The best part was he sat watching it probably because of his Noddyesque costumes without wondering why it was different

The other one was I was walking through town with one of the girls from work (on a day off). She pointed to a Travelodge and said let's go in there. After I suggested that was inappropriate her response was 'Why, it's only a pub' She is one of the few people in the world I could actually believe making that mix up legitimately (she was shocked to discover her boyfriend of about 6 weeks was Asian once)

I'm bloomin' sure I have - hey, I was in software development, and when software developers cock up they do it big style - but nothing springs to mind at present. I'll check with my wife when she gets in. She remembers everything.

We were expecting someone at the surgery where I work once regarding expanding the building and at the appointed time a guy turned up with briefcase saying he was here to see Dr T. He signed in and I asked him if he wanted tea or coffee. The doctor was still seeing patients so I showed him round the building just so he could get an idea of size etc. I then asked him if he wanted to see the Deeds for the building. He said "not really I've only come to see Dr T about my hernia"!!
