The "mix" question is a good point though, but it's the human drivers that screw it up. I think certain sections of motorway will go fully self-drive first. It's the easiest one to manage from the software perspective. The insurance question is a good one - basically it goes to basic 3FT for everyone in a fully self drive world as all the crash claims would be between manufacturers. Anything not part of the self-drive network causes problems, though. So that's non-self-drive cars, people wanting to cross the road, cyclists, people not wanting to cross the road, that cat there, the bus, oh look whatever that is ... It is impossible to test anywhere near every possible driving scenario and the amount of possible issues with them is mind boggling. Sticker or scratches on street signs being able to confuse them shows how far off this technology is. More worryingly they would be easy prey for hackers.
