I have, belatedly, caught up with the news about the police officer shot in a police station.

If I have read the reports correctly, the suspect was in handcuffs yet managed to shoot the officer and himself. I really can’t understand how this has happened. Is there a particular facet of this news story I have missed?

That's the key thing in the reports that makes no sense on the face of it. So I have to assume there's an element that we're not being told for whatever reason.
